Overview
To assist in the early identification and assessment of client's needs through high risk screening and psychosocial assessments and implement timely discharge plans that ensures client continuity of care and effective use of hospital resources.
To coordinate the client's discharge by collaborating with the client, family and multidisciplinary health care team and liaise/advocate for service and change with community service providers and other hospitals.
To enhance the delivery of comprehensive health services to clients and their families through resolution of their psychosocial problems related to management of their illness, medical services and rehabilitation.
