Patients Served
Individuals who have completed their stay in an acute care bed but still require hospital support to meet their goals through an anticipated slower recovery program. They generally have medically complex care needs that cannot be addressed in the community and require further intervention before discharge.
Overview
The unit consists of 10 designated beds on a 20-bed inpatient unit. A multidisciplinary approach to care is provided through a range of disciplines available on the unit or from resources throughout the hospital or community.
The goal of the Complex Continuing Care Program is to assist the patient to achieve his/her optimal level of independence through a collaborative multidisciplinary team process in preparation for discharge. Active participation of the patient and family is an integral component of the program.
