Outreach is an integral part of the CREU Program. CREU is a portable specialized, multidisciplinary team that serves the individual with SPMI in situ, while working very closely with patients, families, and care providers. We serve the same population and region as the inpatient services. Through the CREU Referral Process, we have the ability to identify, which resources would be beneficial for individual patients, whether they be found in our Continuing Care Clinic or Routine Follow-up services.
We are able to provide these services by traditional travel to the communities, teleconferences, and /or videoconferencing. Additionally as part of our outreach services we attempt too build clinical capacity at a local level by informing and providing educational workshops on the latest evidenced based best practices in the field of mental health.
Routine Follow-Up
Patients who have established community supports and good relationships will typically be followed up at one, three and six months to evaluate the effectiveness of the care plan and ensure community reintegration is established prior to final discharge from CREU.
Continuing Care Clinic (CCC)
Community agencies and caregivers have requested more involvement of the CREU Outreach team for some patients at the time of discharge and throughout the six-month follow-up period. As result, CREU has developed a more intense level of service than the one, three, and six-month routine follow-up. Identified as "Continuing Care Clinic (CCC)", patients in the community, the agencies, families and mental health workers who support them, can liaise and consult on a regular basis with the CREU team. The goal is to provide education and support to the community stakeholders, caregivers and patients until relationships are established ensuring a smoother transition for those whose needs are more complex or who are new to the community and support service system.
